Appealing to the Court

If you disagree with the Claims Administrator’s determination of your claim (as communicated by the Claims Administrator in its determination letter to claimants), you have the opportunity to appeal to the Court. Instructions for filing such an appeal are included in the Claims Administrator’s determination letter to you.

If you want the Court to resolve your dispute, you must submit a written appeal to the Court within 21 days of the Claims Administrator’s determination letter. The mailing address of the Court is:

If you submit a written appeal to the Court, please be sure to reference the Case Name and Number: Blackwell v. Mary Black Health System, No. 2017-CP-42-00219. You should also include your Claim ID which is listed at the beginning of the Claims Administrator’s determination letter.
